Sending A Prescription
Please remember that we must receive a veterinarian's prescription before we can
fill your pharmaceutical order.
- Your veterinarian can call in the prescription by phone: 1-800-431-4194
- Your veterinarian can fax the prescription directly to the pharmacy: 1-800-431-4179
- You or your veterinarian can mail in the prescription to:

*Please note that in accordance with state laws we are only able to accept faxed-in prescriptions that come directly from a veterinarian*
This page is not a prescription, but may be useful to your veterinarian in writing
your prescription. You might want to print out this page for your veterinarian's
use.
Your prescription must include the following information:
- Client Name
- Client Address
- Client Phone Number
- Patient Name
- Pharmaceutical Prescribed
- Dosage
- Refill Quantity (up to 1 year*)
- Veterinarian's Signature
- Veterinarian's name
- Clinic Name & Address
- Clinic Phone & Fax Number
- License State and Number
*Ask your veterinarian to indicate a full year’s worth of refills to avoid uninterrupted
service
